Agencies and EMS providers are reminded that a WREMAC Provider Privilege Application (PPA) must be completed and filed with your region’s EMS Program Agency (see below) when any of the following occurs:

1. A provider attains certification for the first time (i.e. someone becomes certified as an EMT); or

2. A provider attains certification at a new level of care (i.e. an Intermediate becomes certified as an AEMT); or

3. A provider, who is already NYS certified, joins or becomes employed by your agency

It is also important to remember that providers must possess and maintain valid CPR certification.  Advanced levels must maintain continuous certification in programs, specific to their level of care (i.e. ITLS, PHTLS, ACLS, PALS, PEPP, or EPC) – please review WREMAC Policy #2013-1 (below) for a list of requirements by level of care.

Provider Privilege Applications are to be filed with the EMS Program Agency before the provider is eligible to go “on-line” with the agency.  The regional EMS Program Agencies are as follows:

COUNTIES: Niagara, Orleans, Genesee – Lake Plains Community Care Network – www.lakeplains.org

COUNTIES: Erie, Wyoming – Office of Prehospital Care – www.OPCEMS.org

COUNTIES: Chautauqua, Cattaraugus, Allegany – Southern Tier Health Care System – www.sthcs.org/stems
